FORMS AND WAIVERS

When you arrive, we will have you sign a hard copy of our forms and waivers for our files. If you need to reschedule or cancel your appointment please call us at (217)342-9355 at least 24 hours before your scheduled appointment.

CANCELLATION POLICY:
*Our services are reserved especially for you. We have a 24 hour cancellation policy. Should you need to cancel your appointment less than 24 hours prior to its start time, a fee of 50% of the full price of service will be charged to you. "No call, no shows" will be charged 100% of the services scheduled. If you are coming in with a certificate for services, the certificate will be marked as used. Please arrive no later than 5 minutes ahead of your scheduled appointment. Late arrivals may result in a shortened or forfeited appointment, as we can not adversely affect other customers. If you are more than 10 minutes late we will have to reschedule your session or delay the next one. If you are 5 minutes late and you have not called us to let us know, we reserve the right to give your float room away to walk-in clients. Nobody wants that, so please be on time and give us at least 24 hours notice if you need to cancel or modify your booking for any reason. Please respect our time as we will always respect yours!

FLOAT EFFINGHAM LIABILITY WAIVER & PROFESSIONAL AGREEMENT:
We want you to have an amazing and safe experience with us at Float Effingham and request that you be aware of and agree to the following information and policies:

Facilities: Amenities provided include: robe, towel, washcloth, ear plugs, shampoo/body wash, and shower. It is up to each individual to take caution to prevent slipping or falling as floor surfaces may be wet. The facility is cleaned between each session. Additionally, the tank is fully filtered and sanitized between each session in accordance with the floatation tank community standards and will uphold all standards of the Floatation Tank Association.

Fees: Fees vary based on the desired service package. These fees are subject to change. We require payment for services at the time of service.

I will NOT use the float pod if:

  • I have not showered thoroughly and still have oils, creams, or makeup on my body.
  • I have had any type of hair color/treatment within the past week.
  • I have had a spray tan in the past 7 days
  • I am under the influence of alcohol or drugs.
  • I have a communicable or infectious skin condition, disorder, or diseases.
  • I have open sores.
  • I am diabetic, unless my diabetes is under medical control.
  • I have a history of heart trouble, epilepsy, seizures or blackouts and have not received my doctors permission to use the float pod.
  • I am experiencing a heavy menstrual period or external vaginal episode.
  • I have a condition which may be adversely affected by cutaneous absorption of magnesium.
  • I have kidney disease.
  • I have incontinence, or voluntarily/involuntarily release of bodily fluids of any kind.

I understand that violation of any of these rules that results in contamination of the float pod water may result in a salt replacement fee of up to $1,000.
I am choosing to use floatation therapy of my own free will and will not hold the owner/operator or Float Effingham liable for any injury during a session or while on the premises. Float Effingham reserves the right to refuse service to anyone.
I have read, understand and agree to all of the terms & policies listed above. This signed document represents an agreement between us.
